I have read the studio policies (see below) and agree to all the terms and additional fees as they apply to me/my student. I agree to pay the monthly invoices on time as outlined in the policies. I agree to the attendance and cancelation policies.

Any personalized exceptions to the studio policies have been discussed and agreed upon with Hannah; this signature agrees to those specifications that pertain to me/my student.
